WORKERS' COMPENSATION

Instructions for Workplace Injury

1.  If injury is life-threatening, immediately call 911, then notify Human Resources at (209) 754-2304 and Employee's Supervisor.

2.  For all other injuries, call the Company Nurse Injury Hotline at (877) 223-9311 and be sure to report injury to Employee's Supervisor.

3.  Appointments should be made outside of Employee's work schedule (if possible) and doctor notes should be submitted to the Human Resources Department.

4. Site secretary/admin will immediately call or email the Human Resources Department advising of the employee's injury and submitting the Supervisor's Report of Injury form.
